Abstract

Currently any health care organization is challenged to improve structure features of its ICUs and create work environment that reduce occupational stress to enhance nursing staff effectiveness and job satisfaction. Objective Assessment of organizational structure feature of intensive care units, and occupational stress for intensive care nurses, and performing intervention for head nurses about strategies for dealing with nurses’ stress. Methods The study was conducted at all intensive care units at four hospitals in Mansoura University, namely; Main University Hospital, Emergency Hospital, Pediatric Hospital, and Gastro intestinal Hospital. The study included all available nurses (173) and head nurses (45) at least had one year of ICU working experience. The collection of data was achieved by three tools 1-Organizational Structure Features 2- Stress rating scale 3- Knowledge test. Pilot study was done and tools validity were 98.30% for tool (1) and 99.25% for tool (2). Result Post program, there were significant correlation at P <0.05 between work place, facilities, layout, ICU culture, team building, nurse to nurse relationship and ICU management with nurse occupational stress. Statistical significant correlation at P <0.05 was found pre and post program between staff organization, nurse to physician relationship, human & financial resources, job satisfaction features of organizational structure and nurses occupational stress. Statistically significant decrease of nurses mean score at P<0.05 in all items of occupational stress. Highly statistically significant improvement of head nurses knowledge in all items of occupational stress and its management strategies post program at p <0.05. Conclusion There were statistical significant negative correlations between physical environment, ICU culture, team building, professional relation and nurses’ occupational stress. Pre program, level of nurses’ occupational stress was high. Post implementation of program, statistical significant improvement of head nurses level of knowledge was found. Also, statistical significant decreases of level of nurses occupational stress in different ICUs. Recommendation Based on the finding, ICU must be staffed sufficiently, provide periodical educational programs, ensure that the workload is equitable with nurses’ capabilities and resources, and clearly define nurses’ roles and responsibilities.
