Abstract

Introduction: Atrial fibrillation (AF) is one of the most frequent arrhythmias in clinical practice and affects 1.5%–2% of the general population. AF induce structural remodelling in the atria by different mechanisms, so evaluation of the left atrium as a cardiovascular biomarker has become increasingly important. LA function can be estimated by two-dimensional Speckle tracking echocardiography that allow us to identify all components of LA function. Aim of the work: The aim of the present study was to assess the functions of left atrium in patients with paroxysmal atrial fibrillation using speckle tracking echocardiography. Subjects and methods: The study was descriptive observational cross-sectional study that was conducted in the department of cardiovascular medicine department, Specialized Medical Hospital, Faculty of Medicine, Mansoura University, in the period from December 2020 to December 2021. The study was conducted on 50 patients with paroxysmal atrial fibrillation. The patients underwent 2D Speckle Tracking Echocardiography study after at least 48 hours in stable sinus rhythm to assess left atrium functions. Results: The study results show that there was a significant positive correlation between strain reservoir and EF (r=0.357, P=0.011) and with Echo (E/A ratio) (r= 0.537, P <0.001), while there was significant negative correlation between strain reservoir and LAAP dimension (r= -0.581, P<0.001). There was a significant negative correlation between strain conduit and LAAP dimension (r= -0.542, P<0.001) and a significant negative correlation between strain conduit and Echo (E/A ratio) (r= 0.628, P <0.001). There was a significant positive correlation between strain pump and EF (r= 0.403, P =0. 0.004), and with Echo (E/A ratio) (r= 0.303, P =0.032), a significant negative correlation with LAAP dimension (r=-0.441, P= 0.001). Conclusion: In patients with atrial fibrillation, left atrial mechanical functions assessed by 2DSTE including reservoir and pump strains have been positively correlated with EF and Echo (E/A ratio) and negatively correlated with LAAP dimension moreover, strain conduit showed a significant negative correlation with Echo (E/A ratio) and LAAP.
