Abstract

Abstract
Background: Female patients with type 1 diabetes mellitus (T1DM) may display menstrual disturbances, hyperandrogenism or early menopause at certain moment of their life, thus representing a significant health problem.
Objective: To detect the frequency of delayed puberty and ovarian insufficiency among a cohort of Egyptian adolescent females with T1DM and to identify it’s correlation with various clinico-laboratory parameters.
Methods: This cross sectional study examined 175 adolescent females with T1DM following up at the Pediatric and Adolescents Diabetes Unit, Ain-Shams University for delayed puberty.
Results: Delayed puberty was found in 6.3 % of the studied cohort. The mean age at menarche was 13.2 years and the percentage of Tanner V was 28%. The percentage of ovarian insufficiency was 36.6%. The median serum leptin was 5 ng/ml (range 1-18). Ovarian insufficiency was correlated with mean of age, duration of DM, waist circumference Z score, waist/hip Z score, serum leptin and low density lipoproteins (LDL).. Multivariate logistic regression analysis showed that duration of DM ˃ 9 years)p-value:0.029), waist/hip ratio ˃ 0.75(p-value:0.004), LDL˃129 mg/dl(p-value:0.034), serum leptin˃3 ng/ml(p-value:0.009) were the most important factors associated with ovarian insufficiency .
Conclusion: Ovarian insufficiency is a prevalent morbidity among adolescent females with T1DM. It is associated with obesity, insulin resistance, and leptin among adolescent females with T1DM
