Abstract

SUMMARY
hildhood obesity is a worldwide challenging condition
with multifaceted aetiology and implies ongoing health
risks on many aspects.
Physical fitness has been shown to be a major health factor
in both children and adolescents. One of the most widely applied
physical fitness test batteries in Europe is the Alpha-fit Test. The
higher levels of muscular strength may have a positive influence
on psychological distress (i.e negative affect and stress) and
psychological well-being indicators (i.e self-esteem and
optimism), as well as higher levels of cardio respiratory fitness, in
overweight/obese children and adolescents.
Co morbid psychological and emotional disorders of
obesity generally act as causal or maintaining factors of obesity
and thus significantly affect the treatment outcome. These
disorders comprise a wide range of conditions as depression,
anxiety, eating disorder, stress, body shape concerns, low selfesteem.
In this study, we aimed to assess physical fitness i.e
(cardio-respiratory fitness, muscular strength and speed-agility)
in children with simple obesity as well as the association of
physical fitness with behaviour problems and health related
quality of life.

This Cross-sectional case control study was conducted at
tertiary care hospital at Obesity Clinic, Pediatric Hospital, Ain
Shams University, from October 2021 till August 2022 and
performed on 101 Patient with simple obesity aged (5-17) years
old following up regularly in Obesity Clinic and 53 age and sex
matched normal children as a control group. Psychometric
evaluation as Child behaviour checklist (CBCL) to assess
behavioural changes and Health related Quality of life measure
were done. Physical fitness assessment including 20m
endurance shuttle run, Handgrip test, Sit & reach flexibility
test, Flamingo balance test and Alpha fitness test were
measured.
Cases group had more females and less paternal
consanguinity history with statistically significant difference
between two studied groups as p-Value was <0.05. The
anthropometry as child BMI and Waist/Hip ratio were
increased in cases with statistically significant difference
between two studied groups. The percentage of metabolic cases
were increased in cases group with statistically significant
difference between two studied groups in acanthosis negricans
only.
Regarding the alpha health related fitness test, hand grip
strength, 20m run, skin fold (Tri) and skin fold (S.scap) were
increased in cases group, while standing long jump was
decreased with statistical significant difference between two
studied groups as p-Value was <0.05.Summary 

As regards psychiatric assessment, Clinical cases of
affective, anxiety and somatic were increased in cases group.
Borderline cases in attention, opposition and conduction were
also increased in cases group without statistically significant
difference between two studied groups in any of them.
We concluded that Obesity has a negative impact on the
children’s daily life. Obese children reported poorer quality of
life as regards the physical, emotional, social, school
functioning domains and total quality of life than normal
children. Management of obesity should include health related
quality of life measurements as a parameter of overweight and
obesity outcome.
