Abstract

Abstract
Background: Septic shock is one of the leading causes of death in Intensive care unit, although survival has been improving, its morbidity and mortality are substantial and arise through the consequences of the process on vital organ function.
Aim of the Work: to compare the effect of norepinephrine alone and Norepinephrine/ vasopressin combination on hemodynamics and tissue perfusion in septic shock patients.
Patients and Methods: This Study was done on 90 adults of either sex in intensive care unit who developed septic shock. Patients were randomly divided into two groups, according to the drugs used, by the single blind technique as the treating physician was the only one is aware of the drugs used group 1: 45 patients received IV Norepinephrine infusion starting with (0.1mcg/kg/min). group 2: 45 patients received IV Norepinephrine infusion Starting with (0.1 mcg/kg/min). +Vasopressin infusion at the rate of (0.03 unit/min).
Results: Mean arterial blood pressure was significantly higher in Norepinepinephrine +Vasopressin group (Mean: 75.26±1.31 mm Hg) compared to Norepinephrine (Mean: 74.62 ±1.50mm Hg) and with P-value 0.003, Central venous oxygen saturation was significantly lower in Norepinepinephrine (68.15 ± 3.88) Compared to Norepinephrine +vasopressin group (69.73 ± 3.02) ,P-Value: 0.034, NGAL was significant higher in Norepinephrine group (Mean: 171.98±10.69 ng/ml) , Compared to Norepineprhine +Vasopressin group (Mean: 167.79±9.15 ng/ml) P-Value: 0.047.
Conclusion: Use of Vasopressin with norepinephrine was associated with significant reduction of the heart rate, improvement of mean blood pressure and with improvement of Tissue perfusion parameters and improvement of the kidney function as regards Urea, Creatinine and Even early kidney Injury Biomarker (NGAL). However Use of Vasopresin has no effect on improving the inflammatory biomarkers.
