Abstract

This study aimed to evaluate the effect of emergency obstetric protocol on the performance of intern nursing students. A quasi- experimental study carried out at technical institute of nursing Ain Shams University and in emergency department, labor and intensive care unit in maternity hospital Ain Shams University. A convenient sample was used (100) female intern nurse in the technical institute of nursing, Faculty of Nursing, Ain- Shams University in the academic year 2019/2020 and a purposive sample of (59) conscious women with obstetric emergencies who agreed to participate in the study and with non co- morbid condition from emergency unit, intensive care unit and labor unit of maternity hospital Ain- Shams University was obtained.
A pilot study had been conducted on 10% of total sample size, 10 intern nurse students were under study to evaluate applicability and reliability of the constructed tools. Data had been collected within 6 months. The researcher attended for five days/week from 9 am to 1 pm for two weeks in technical institute of nursing (for knowledge and practical session).
The researcher went to the maternity hospital in pervious mentioned units two days per week from 9.00 A.M to 1.00 P.M; the time for evaluation and follow up was extended for six months starting from the beginning of October 2019 till end of March 2020.
Four tools utilized for data collection:
1- Self-administered structured questionnaire sheet: which included two parts to assess general characteristics of the study sample and intern nursing students’ knowledge regarding obstetric emergencies.
2- Obstetric triage tool: to assess and evaluate intern nurses’ practical skills for obstetric triage to prioritize the women’s urgency for providing care.
3- Observational checklist: to assess and evaluate practical skills of intern nursing students regarding obstetric emergencies care.
4- Maternal satisfaction questionnaire sheet: to measure the women’s satisfaction level regarding emergency obstetric care provided by intern nursing students after protocol implementation.
Main results of the present study were as follows:
 There was statistically significant difference between pre, immediately post protocol intervention and in follow up phase related to intern nurse’s knowledge and practices about emergency obstetric care.
 Percentage distribution of the studied intern nurses according to their total satisfactory knowledge scores related to emergency obstetric care pre, immediately post protocol intervention and in follow up phase was 27%, 93% and 87% respectively.
 Percentage distribution of the studied intern nurses according to their total competent practices scores related obstetric triage pre, immediately post protocol intervention and in follow up phase were 24%, 86% and 90% respectively.
 Percentage distributions of the studied intern nurses according to their total competent practices scores related to obstetric emergencies care pre, immediately post protocol intervention and in follow up phase were 26%, 92% and 92% respectively.
 There was statistically significant difference relation between intern nurses’ knowledge, practices related to obstetric triage, emergency obstetric care post intervention and their qualifications (previous experience and training program).
 There was significant positive correlation between intern nurses’ knowledge score and intern nurses’ practices score about emergency obstetric care post intervention.
 Total women’s satisfaction scores with nursing care provided by intern nurses after implementation of emergency obstetric care protocol as 71.1% of women were satisfied while 28.9 % were dissatisfied.
 There was statistically significant positive correlation between total intern nurses’ practices, knowledge related emergency obstetric care and women’s satisfaction post intervention of protocol.
Based on findings of present study, it can be concluded that:
Implementing of emergency obstetric protocol had a positive effect on the performance of intern nursing students. Moreover, there was a statistically significant correlation between total knowledge and total practice scores of intern nursing students post protocol implementation and increased women satisfaction level regarding nursing care provided by intern nursing students. The present study supported hypothesis and the aim of the study.
Based on the findings of the present study, the following recommendation is suggested:
• Dissemination of emergency obstetric protocol for all intern nursing students in technical nursing institutions for improving knowledge and practical skills of intern nurses about obstetric emergencies care.
Further Researches:
• Implementation of training program for nurses related emergency obstetric care at maternity hospital Ain Shams University.
• Teaching institutions as technical institute of nursing should work in collaboration with hospital for facilitating students’ learning, achieving educational goals to improve quality of care and increase patients’ satisfaction.
• Further research is recommended to investigate the barriers that affect performance of intern nurse during internship period in obstetric and gynecological departments.
• Replication of the study on large sample size and in other different institutions for generalizing the findings.
