Abstract

Cervical-vaginal infections are one of the most common problems in clinical medicine. Gardenella vaginalis, Candidiasis and Trichomoniasis are responsible for 90% of the cases of diseases infectious origin. Other types that sexually transmitted like chlamydia trichomatis.
The assessment of vaginitis should be performed by accurate history talking, clinical examination and other microscopic tests.
The cytological diagnosis (Pap-smear) was used for screening of cervical cancer, in addition, G. vaginalis, T. vaginalis and C. albicans infections can also be easily diagnosed from cervical cytology, by identification either of the organism or of characteristic cytological cellular changes.
In current study, 400 patients who are at reproductive period with cervical-vaginal infection.
All patients were attending the gynecology, infertility and contraceptive clinics in Ain Shams University Maternity Hospital. History and clinical examination were taken from all patients, non lubricated vaginal speculum was inserted. The cervix, vagina, vaginal wall and fornices were carefully inspected for the presence of any pathological lesion, and then cervico-vaginal smear was collected. All slides are fixed in 95% concentrated alcohol which used for fixation. The results of Pap smear were done.
The of the study reports of different variables to cervical vaginal infections show that 20.5% of the studied subject had no infection while 73.3% had positive single infection and 6.3 % had mixed infection.
According to single infection, it showed that 28% of the subjects had chlamdyia infection, 18% had gardinella infection and 39% had candida infection.
However, according to mixed infection; it showed that 64% of the mixed infections were chalmydia and candida, 24% of the mixed infection were candida and gardinella while 12% of the mixed infection were chamyldia and gardiinella.
The results of Pap test are widely variable. The organism should be reported when identified. The less and specific tests can be combined with a confirmatory test as wet mount examination or culture or PCR, if necessary, to provide the best results for the patient.
The current study recommends cervico-vaginal smear should be taken at 21 years or within three years of marriage for cervical abnormality and for the prevalence of causative organism of cervico-vaginal infection. Liquid based preparation (Thin Prep) can be used with advantage over Pap test. Cases with cytological Pap smear abnormalities should confirmed either with colposcopic examination or DNA testing assay.
