Abstract

The increasing incidence of morbid obesity suggests that the quantity of bariatric surgical procedures will continue to multiply each year. Bariatric surgery has become an accepted approach to weight management with the additional benefit of resolution of several co-morbidities. The nurse should see that the patient has appropriate referral information for support services. Clearly the patient and family need to go home with specific information about nutrition, exercise, skin, wound care, ambulation, self-care, signs and symptoms that require medical attention (Pieracci, Pomp & Barie, 2016).
Ideally the nurse, patient, and family should have an opportunity for education sessions prior to discharge. During the postoperative period, nurse can serve as a source of information and a monitor of patient progress. Follow-up to patients and families after surgery are very helpful in determining the welfare of the patient if any complications are developing. Nurse should assess psychological condition of the patient. Nursing staff should be supported for caring bariatric patients to ensure policies and protocols are adhered to and a high quality of evidence-based care is delivered (Martinez, 2014 and Grindel & Grindel, 2006).
Aim of the Study:
This study aims to:
Assess nurses’ performance for patient undergoing bariatric surgery through:
1- Assessment of nurses’ knowledge regarding management of patient undergoing bariatric surgery.
2- Assessment of nurses’ practice regarding management of patient undergoing bariatric surgery.
Research Questions:
1- What is the nurses’ level of knowledge regarding management of patients undergoing bariatric surgery?
2- What is the nurses’ level of practice regarding management of patients undergoing bariatric surgery?
Subject and Methods:
Research design:
A descriptive exploratory design was conducted to achieve the aim of this study.
Setting:
This study was conducted in surgical units at Ain Shams University Hospital affiliated to Ain Shams University Cairo-Egypt.
Sample:
A purposive sample of nurses working in surgical units at Ain Shams University Hospital.
Inclusion criteria:
• Nurses already dealing with patients undergoing bariatric surgery.
• Years of experience not less than one year for working in surgery units.
• Nurses who are willing to participate in the study.
Tools of data collection:
The data were collected through using the following tools:
I- A structured self-administered knowledge assessment questionnaire:
The self administered questionnaire was used to assess nurses’ level of knowledge regarding management of patients undergoing bariatric surgery.
II- Evaluation practice checklist To assess nurses̕ level of practice regarding management of patient undergoing bariatric surgery.
The present study revealed that:
Regarding gender, the results revealed that (66.7%) of them were females. As regards to age, (66.7%) of them were >30 to 40 years old. In relation to educational level, (56.7%) of them were nursing diploma. While regarding the years of experience (66.6%) of them had experience more than 15 years. Regarding training courses and performance enhancement (76.7%) of them didn’t attendance any training courses and six of them represented (85.7%) had performance enhancement regarding management of patient undergoing bariatric surgery.
Concerning the study nurses performance regarding management of patient undergoing bariatric surgery, (73.3%) of them had unsatisfactory knowledge, (70.0%) of them study nurses had unsatisfactory practice.
There was a significant correlation between total nurses’ knowledge and total nurses’ practice (r=0.398 at p˂ 0.05).
Conclusion
The study revealed that about three quarter of the studied nurses had unsatisfactory knowledge. While more than two third of the studied nurses had unsatisfactory practice regarding management of patient undergoing bariatric surgery. Moreover, there was a statistically significant correlation between studied nurses’ knowledge and practice. In addition to, there was statistically significant correlation between studied nurses’ knowledge, practice and their demographic characteristics.
Recommendations
• Replication of the current study on large sample and different hospitals settings to be able to generalize the results.
• Further study is recommended to evaluate the reflection of educational training program regarding management of patients undergoing bariatric surgery on nurses’ performance and consequently on the patients outcomes.
