Abstract

45 patients with proposes were examined between 1999 and 2001 in Minia University Hospital. The patients were 23 males and 22 females with age ranging from one to seventy years with mean 25.53 years. 35 patients had unilateral proptosis and 10 cases had bilateral proptosis. All patients were subjected to:
History talking
Local and general examination
Plain X-ray study of the orbit.
A-scan and B-scan orbital echography
Axial and coronal orbital CT scans
Other studies: MR imaging of the orbit, examination of the filed of vision, color vision testing, excisional and incisional biopsies, T3 and T4 level, complete blood picture, sternal puncture, chest X ray, abdominal us, therapeutic steroid test and endoscopic examination of paranasal sinuses were done in some patients guided by the results of clinical evaluation and primary studies. It was found that inflammatory causes of proptosis were the most common followed by neoplastic, congenital/structural, traumatic and then vascular cases. Inflammatory orbital pseudotumor was the most common cause followed by, mucocele of the paranasal sinuses and thyroid orbitopathy.
Clinical evaluation provided accurate diagnosis in 16 cases, differential diagnosis in 15 and failed to give accurate differential diagnosis in 14 cases. The sensitivity of clinical evaluation in this study was 53.3% its accuracy was 35.56% and its+ve predictive value was 51.6%.
