Abstract

Vascular endothelial dysfunction was proved to be the leading ause of increased mortality in the diabetic population. It is not known whether diminished ED RF responses is due to insulin deficiency and/or high glucose levels which are known to cause endothelial damage. In a study to test for the effect of basal insulin on vascular resistance in humans, 22 normal healthy volunteers were studied in 3 groups. The 1 st group of 8 persons were studied for the effect of absence of basal insulin on vascular resistance and endothelial . dependent vasodilation, this was done by infusion of somatostatin. Skeletel muscle leg blood flow response to methacholine infusion was blunted by more than 500/0 during somatostatin with doubling of the circulating free fatty acids. In the second group of 8 persons, after replacing the circulating basal insulin by insulin clamp technique, the response of endothelial dependent vasodilation was normalized with return of the free fatty acids to the previous levels.
