Abstract

This study was designed to evaluate the success of implants installed in previously distracted alveolar bone in anterior mandibular area using extraosseous distractor device (TRACK by Martin KLS). This study was conducted on eight patients with vertically deficient alveolar ridge in anterior mandibular area who were examined and managed at the Department of Oral and Maxillofacial surgery, Faculty of Dentistry, Tanta University. Surgery was done for all patients in three stages: - • Stage I: Distractor device fixation. • Stage II: Removal of distractor device and implants installation. • Stage III: Implant exposure and abutment fixation. All patients were evaluated preoperatively: 1- Clinically to evaluate oral hygiene, occlusion and the condition of the neighboring teeth. 2- Radiographically by using panoramic x-rays to measure the height and width of residual alveolar bone and to detect underlying intra-bony pathology if present. 3- Study casts to evaluate occlusion, the inter-arch space and the length of edentulous span. The results: - Stage I (Distractor device fixaton): - A) Clinically: - All patients were examined one week after the surgery 1): Wound healing was uneventful in all patients except case NO:2 who showed dehiscence at the surgical site which healed by secondary intension at the third week. 2): All patients showed stable distractor device through distraction and consolidation period. B) Radiographically: - 3D (C.B.C.T) scans were taken after three months at the end of consolidation period and before device removal and implants installation 1): There was significant increase in height of the alveolar ridge when compared to the preoperative values. 2): The newly formed bone between osteotomies in regeneration chamber showed accepted integration with the surrounding native bone. Stage II (Removal of distractor device and implants installation): A) Clinically: - 1): All patients showed increased height of the alveolar process compared to their previous situation 2): All implants were installed with adequate primary stability measured by implant torque wrench. B) Radiographically: - Panoramic x-rays were taken for all patients showed implants engaged in both coronal native bone and apical newly formed bone. Stage III (Implants exposure and abutments fixation). 1): All implants in all patients were osseointegrated and evaluated by OSSTELL device. 2): Abutments fixated prior to prosthesis delivery.
