Abstract

The aim of periodontal treatment is ‘prevention of progression of periodontal disease and regeneration of the lost periodontal tissues.
Periodontal treatment should result in resolution of inflammation, stop the progression of periodontal disease, enhance pink esthetics and produce and maintenance of periodontal health.
Though several surgical techniques have been constantly proposed and experimented, they are mostly invasive and unpredictable. Moreover, the success rate of surgical augmentation of papilla relies on the thickness of gingiva biotype. Hence, a number of nonsurgical, The main minimally invasive techniques have been developed to preserve and restore interdental papilla. Though many solutions have been proposed to correct lost interdental tissues, no golden standard technique is followed so far due to the absence of long-term clinical results and predictability.
Platelets concentrate is considered as important reservoir of various growth factors and cytokines, which play a vital role in inflammation, wound repair, homeostasis and antimicrobial host.
Recently, PRF was developed due to many growth factors that are released which aid in healing process. The released growth factors are such as (IGF I-II), (PDGF), (TGFβ-1) and (VEGF). I-PRF has been used due to presence of higher concentrations of growth factors and stem cells which can be observed when compared to other formulations of PRF that could facilitate wound healing and bone regeneration.
HA gel injection of the interdental papilla showed successful results in many previous studies, however more clinical trials are recommended on a larger sample size with different concentrations of HA and longer follow up periods.
So, this study was performed to compare between the clinical and radiographic effectiveness of L-PRF and HA gel in IDP reconstruction in class I and II papilla deficiency.
Both groups showed significant improvement in clinical parameters and patients’ satisfaction after 3 and 6 months of follow up.
from the present study it can be concluded that:
1- Both L-PRF and HA gel were effective in IDP reconstruction and showed marked improvement in all measured clinical parameters through the 3 and 6 months follow up.
2- The improvement in the mean of all clinical parameters was comparable between L-PRF and HA groups through the follow up periods.
3- HA gel group had overall higher patient satisfaction compared to L-PRF group. Therefore, our study demonstrated that interdental papilla reconstruction using Hyaluronic acid gel injection can be a successful treatment option for interdental papilla deficiency.

Recommendations
1- Studies with longer periods of follow up and larger sample size should be done to evaluate the effect of multilayer L-PRF and HA gel in IDP reconstruction.
2- Studies on the evaluation of the effect of single versus repeated surgical procedures on the relapse of reconstructed interdental papilla should be carried out.
3- Studies to evaluate the effect of different injection protocols and different concentrations of hyaluronic acid in IDP reconstruction are recommended.
4- The efficacy of HA on reconstruction of IDP is still limited with the small number of studies available in the literature, so more clinical trials should be performed to support the scientific evidence.
