Abstract

T
here are many modalities available to achieve successful fusion with few complications following ACDF, our study aimed to compare the outcome of anterior cervical discectomy and fusion by cage alone versus surgical fusion by cage and plate with regards to fusion rates and incidence of cage subsidence to assess the superiority of one method over the other and justify the cost effectiveness and length of operative time as well as incidence of complications.
While the results showed that there was no significant difference between using cage alone vs cage and plate in achieving fusion and the incidence of post operative dysphagia, there was a lower incidence of cage subsidence in the cage and plate group 12.7% vs the cage alone group 34.1% however this is at an added cost of the plate and increase in operative time to install the plate.
Further randomized controlled trials are recommended with longer follow up periods, to assess the difference in quality of life indices with objective scans which facilitate accurate comparison and subsequently decision, as these were not clearly described in the available studies. Therefore, we recommend putting it into consideration in the future as the presence of which would greatly add to the current evidence base and would be a practical, and clinically applicable approach and would help in overcoming some of the issues. Also to compare with the different emerging hybrids like stand-alone cages and also with dynamic fixations and artificial discs.
