Abstract

Subtrochanteric fractures in paediatrics which is defined as a fracture that is located within 10% of the total femur length below the lesser trochanter.
Closed reduction and spica casting are the treatment of choice under 5 years. In adolescent intramedullary nail with trochanteric entry is the preferred treatment. In the age group from 5 to 12 years old the optimal way of treating subtrochanteric fractures is still controversial.
The aim of this study was to Evaluate union rate and fracture alignment on radiographs, determine optimal time of full weight bearing & Identify complications associated with elastic nailing of subtrochanteric femur fractures in paediatrics.
This study started at January 2020. We included 20 children aged from 5 years till 12 years old with Subtrochanteric fracture of femur. Our data suggest that fixation of subtrochanteric fracture in pediatrics by elastic nails is a safe and reliable method, and maintains good fracture alignment. The mean NSA on the fracture side was 138.65±6 compared to a contralateral angle of 141.45±6.4, with a statistically insignificant mean angle difference of 2.8±2.76. the technique is minimally invasive with no disruption of the periosteum and fracture hematoma. One patient (5% in this series) complained of the painful prominent nail ends that required shortening.
