Abstract

Background: Obstructive sleep apnea (OSA) constitutes the most important subgroup of sleep-disordered breathing (SDB) in which cessation of breathing occurs during sleep (apnea) and symptoms like snoring and daytime sleepiness are frequent. OSA is associated with serious adverse events for affected individuals such as myocardial infarction, hypertension, stroke, and traffic accidents.Polysomnography and Drug-induced sleep endoscopy represent now the main tools for not only pre-surgical evaluation of OSA patients (particularly; severity of apnea and areas of collapse) but also for post-operative follow-up and evaluation of different surgical techniques. Aim of this study: This study is to evaluate the role of trans oral drug induced sleep endoscopy (DISE) in evaluation of tongue palate interaction and its impact on surgical outcome. Method: This study is a prospective interventional study including 42 Egyptian ; 24 male (57.1%) and 18 females (42.9%) and, their ages ranged between 25 to 54 years.Lateral pharyngoplasty was done to all cases in study 6 months postoperative ,Sleep study (Polysomnography) was done to assess the AHI and other respiratory parameters and compare the postoperative study with the preoperative one and whether or not AHI was significantly diminished or not. Results: among studied cases 73.8% of the studied cases were severe OSA pre lateral pharyngeoplasty that decreased to 16.7% post lateral pharyngeoplasty and there is 16.7% of the studied cases converted to normal. 61.9% of the studied cases successed after lateral pharyngeoplasty. higher percent of improvement post-surgery among –ve TP interaction group(48.8%) than +ve TP interaction group (25.2%). higher percent of improvement of AHI among –ve TP interaction group (52.8%) than +ve TP interaction group (29.3%). Conclusion: Patients with +ve tongue palate interaction shows poor response to isolated palatopharyngeal expansion and thus needs further treatment plane to improve outcome. Recommendations: A larger number of patients and longer follow up periods are required to evaluate any delayed complication or failure of the operation.
