Abstract

The present study aimed to evaluate adipose tissue and bone marrow derived mesenchymal stem cells therapy for surgically induced knee osteoarthritis in sprague-dawely rats. Totally 30 Sprague dawely rats were used in this study and were divided into 5 groups (6 rats in each group): group (I): (negative control) normal rats without surgical induction of osteoarthritis ,group (II): (positive control) rats with surgical induction of osteoarthritis, group (III): osteoarthritic rats treated with intrarticular injection of bone marrow derived mesenchymal stem cells 4 weeks after surgery ,group (IV): osteoarthritic rats treated with intrarticular injection of adipose tissue derived mesenchymal stem cells 4 weeks after surgery, group (V): osteoarthritic rats treated with intrarticular injection of Phosphate buffered saline (PBS). All rats in each group were euthanized ten weeks after surgery. Clinical assessment of pain through observing voluntary movement of animal and signs of pain and discomfort. In addition to histopathological assessment of cartilage damage according to Mankin’s scoring system in all experimental groups . Safranin-O fastgreen staining was used to evaluate extracellular matrix abnormalities, Masson trichrome staining was used for assessment of cartilage collagen and MMP-9 immunohistochemistry staining using Rabbit polyclonal MMP-9 antibody to display marked expression against MMP-9 for the enzyme matrix metalloproteinase in the articular cartilage in the diseased group in comparison to other groups and calculate number of immunopositive cells per 1000 cells. The results of the present study demonstrate that the rats which received ADSCs showed better cartilage quality and lower degree of cartilage degeneration histopathologically and there was a trend towards higher scores for all parameters in treated groups. and demonstrated that chondrocytes and chondroid matrix regained their histological architecture upon adipose tissue-derived stem cells more than bone marrow, the adipose tissue-derived stem cells is mimicking the normal articular cartilage, in spite of the bone marrow-derived stem cell alleviated inflammatory process and increased the lay down of chondroid matrix and the superficial cracks remains for some degree. In conclusion, the results of the present study demonstrated that ADSCs and BMSCs are capable of down regulating inflammatory factors and alleviating knee OA by its strong regenerative capacity on chondrocytes and increased chondroid matrix deposition .Considering that reconstructive stem cell therapy is becoming recognized in clinics, particularly in orthopedics, this study justified the potential use of ADSCs and BMSCs as a therapeutic modality to impede the pathologic course of knee OA. ADSCs are more effective and easily available compared to BMSCs. Further studies are necessary to: • Clarify the required cellular dose for OA therapy • Determine whether one injection will be sufficient to reach the desired result within a given time • Monitor the in vivo behavior of implanted MSCs in host tissue and to understand the fate of these cells. This will be important for the development of successful cell therapies • Extension of this research to use ADSCs in regenerating cartilage in osteoarthritic patients will be interesting.
