Abstract

This work aimed to control spread of TB by:
• A sensitive and rapid method for the laboratory diagnosis of active tuberculosis disease (Pulmonary and extrapulmonary) in patients.
• A sensitive and rapid method for the diagnosis of latent tuberculosis infection in household contacts of COI1fil‘1I’l6Cl pulmonary tuberculosis patients and healthy community controls.
Eighty six subjects were included. Thirty- six patients with suspected active TB were included in group I (25 males and 11 females with mean age 3l.9i 13.4 and age range 16-60 years). It included 24 patients with suspected pulmonary TB, recruited from Zagazig Chest Hospital and Chest department in Zagazig University Hospitals and 12 patients with suspected extrapulmonary T B recruited from different departments in Zagazig University Hospitals. Twenty eight household contacts of confirmed pulmonary TB patients were included in group II (11 males, 17 females with mean age 28.3± 11.8 years and age range 17-55 years). Twenty two healthy community controls without history of exposure to TB were included in group III (9 males, 13 females with mean age 3l.ld: ll.8 years and age range 17-55 years). All subjects in group 11 and HI were selected to have negative results for Zeihl-Neelsen (ZM staining and culture on Löwnestein-Jensen (LJ) medium.
This work Conclude that :
1- ZN may be still recommended being cheap and rapid. in pulmonary TB, the concentrated stained film is preferred. But, negative results don’t exclude TB and false positive results may occur with NTM.
2- Nested PCR assay on PBMCs may be a useful tool for diagnosis of active TB, especially if it is difficult to obtain a clinical specimen as in case of extrapulmonary TB.
3- ICT is rapid and easy to perform without requiring special equipment. However, the sensitivity demonstrated by the test (Either single or combined with QFT Gold-IT] does not suggest, for the moment, that it should replace the standard methods for active TB diagnosis.
4- Using the results of QFT-Gold IT plus ZN staining could be very helpful in diagnosis of active TB. Positive results in both indicate active TB, and negative results exclude it.
