Abstract

B
one marrow transplant is a procedure used to replace damaged or destroyed bone marrow with healthy bone marrow stem cells. It is the soft, fatty tissue inside bones. Stem cells are immature cells in bone marrow that give rise to all of blood cells. It is used as a strategic treatment for a variety of immune deficiencies: oncological diseases, hematologic, metabolic disorders such as leukemia, lymphoma, myeloma, aplastic anemia, myelofibrosis, myelodysplastic syndromes, thalassemia, germ native cell tumor among other potentially lethal diseases, major and primary cell immunodeficiency (David, 2013).
Nurses play an important role in the care of such group of patients pre, during and post hematopoietic stem cell transplantation process. They should also have proper knowledge and practices to provide proper care and prevent complications. Enhancing their knowledge and practices will help to prepare a planned care plan for improving patients` condition. The most important nursing contributions are patients’ education and psychosocial support (Lynn, 2011 and Molassiotis, 2011).
Aim of the Study
The present study aimed to suggest nursing management protocol for patient undergoing bone marrow transplantation. This aim was achieved as follows:
 Assessing knowledge and practices of nurses caring for patients undergoing bone marrow transplantation.
 Developing nursing management protocol for patients undergoing bone marrow transplantation
Research questions:
 What are the knowledge and practices of nurses caring for patients undergoing bone marrow transplantation?
 What are the relations between nurses’ knowledge and practices as regards suggested nursing management protocol?
Operational definitions:
 Undergoing: means pre, during and post bone marrow transplantation
 Post-transplantation: (immediately after transplantation and ongoing care until the patient is discharged from the hospital).
Research design:
A descriptive exploratory design was utilized for the conduction of this study.
Research setting:
The study was conducted in the Bone Marrow Transplantation Unit at Nasser Institute Hospital.
Subjects
A convenience sample of 60 nurses working in the above mentioned setting was involved in this study.
Tools for data collection:
Tool (I):
I- Self- administered questionnaire (Appendix II):
It was used to assess nurses’ knowledge for caring of patients undergoing bone marrow transplantation.
Tool (II):
An observation checklist (Appendix Ill)
It was developed by the researcher after reviewing the related literature to assess nurses` practices on caring patients undergoing bone marrow transplantation
Results:
The present study revealed that:
- All nurses were female, 36.7% of them ranged from 31-40years of age.
- 48.3% of nurses were diploma nurses, 65% of them had experience ranged from 1-5 years and all of them had training course regarding BMT practice.
- The majority of nurses under the study had unsatisfactory level of knowledge regarding patients undergoing bone marrow transplantation.
- Most nurses’ had satisfactory level of practice regarding care of patient undergoing bone marrow transplantation.
- There was a statistically significant relation between level of nurses` knowledge as regards their age, level of education, years of experience in relation to care of patients undergoing bone marrow transplantation.
- There was a statistically significant relation between nurses` practice as regards their age and years of experience in relation to care of patients undergoing bone marrow transplantation.
- That there was statistically significant relation between overall nurses` knowledge and practice regarding care of patients undergoing bone marrow transplantation.
Conclusion:
Based on the findings of the current study, it can be concluded that, the majority of studied nurses had unsatisfactory knowledge regarding care of patients undergoing bone marrow transplantation with statistical significance difference. Meanwhile the majority of them had satisfactory practice with high statistical significance. Additionally, results revealed that there was a statistically significant relation between total nurses’ knowledge and practice as regards their demographic characteristics. Also, there was a statistically significant relation between total nurses’ knowledge and practice.
Recommendations:
Based on findings of the current study, the researcher recommends the following:
1- Continuous evaluation of nurses’ knowledge and practice is essential to identify nurses’ needs and factors affecting their performance in critical care units.
2- Close supervision and teaching on spot during work is needed to ensure that quality of care is provided by nursing care of patients undergoing bone marrow transplantation.
3- Developing continuous educational programs including evidence based guidelines based on needs’ assessment for nurses to improve their performance and quality of care regarding care of patients undergoing bone marrow transplantation.
4- Developing a simplified and comprehensive educational guidelines and booklet about nursing management for patients undergoing bone marrow transplantation
5- Developing educational program/ self-learning packaging (SLP) for infection control and follow of aseptic techniques during care of such critical patients.
6- Nursing curriculum must include theoretical and practical sessions about management of BMT for different nursing categories (diploma – technical- bachelor).
7- Replication of the study on a larger probability sample and different geographical areas to generalize the results.
