Abstract

The appearance of enamel demineralization areas or white spot lesions is a significant risk associated with fixed orthodontic treatment in the absence of proper oral hygiene. This work aimed to evaluate and compare the effect of different topical agents utilized for prevention of enamel decalcification around orthodontic brackets on Bleached Vs Non-bleached Enamel Surfaces. This study was carried out on one hundred and twenty extracted human premolars with intact buccal surface. The teeth were divided into 2 equal groups. group I: non- bleached group, group II: The teeth of this group were subjected to bleaching. Metal premolar brackets(NANDA, Ortho Organizers, Carlsbad, California, USA) were bonded on the buccal surface of both groups using a light cured composite resin and adhesive (Transbond XT, 3M, California, USA) in accordance with the manufacturer’s instructions. group I and II were subdivided into six subgroups (n= 10 for each one A, B, C, D, E, and F) according to the materials utilized for prevention of enamel decalcification around orthodontic brackets. All the materials were used according to manufacturer’s instructions. DIAGNOdent pen and EDAX attached to ESEM were used to measure the level of mineralization of dry enamel surface before utilizing the topical agents. All teeth were cycled in demineralization solution for 8h/day and artificial saliva approximately 15h/day for 15 days. In the end of 15th day all teeth were rinsed with tap water and the same methods and procedures were used before the cycle and application of the topical agents repeated to evaluate the demineralization and effect of all the agents to prevent it. The data were statistically analyzed. Results: Regarding the non-bleaching subgroups; all studied material revealed significant demineralization reduction in comparison to the control subgroup (P<.05). Ortho-Choice Ortho-Coat revealed the highest significant effect while GC Tooth Mousse showed the least effect. In bleached subgroups; Profluorid varnish, Enamel Pro Varnish and Ortho-Choice Ortho-Coat significantly reduced demineralization (P<.05) while either GC MI Paste Plus or GC Tooth Mousse had no significant effects (P>.05).
