Abstract

Diagnosis of patchy hair loss in pediatric patients is often a matter of considerable debate among dermatologists. Dermoscopy can be viewed as rapid and non invasive tool to detect the cause of patchy hair loss. Like clinical dermatology, dermoscopy works parallel to the skin surface and perpendicular to the histological plane; like the histopathology, thus it allows the viewing of structures not discernible by the naked eye.
This study included 40 patients (20 patients with tinea capitis and 20 patients with alopecia areata); they were examined clinically, lab investigations done as direct microscopic examination with 10% KOH and fungal culture to confirm clinical diagnosis and dermoscopically, aiming at finding the different dermoscopic features of TC and AA.
Our obtained results reported that, in TC patients the most common dermoscopic feature was short broken hairs followed by black dots, but both of them non specific as they were detected in other conditions of hair loss. Comma shaped hairs, corkscrew hairs, and zigzag shaped hairs are the diagnostic dermoscopic features of TC.
Our obtained results reported that, in AA patients, the most common dermoscopic feature was black dots but it is not specific for AA, as it is found in other conditions as trichtillomania and TC. But the specific feature was yellow dots, exclamation mark and short vellus hairs.
Dermoscopy can be used to differentiate between TC and AA by detecting their characteristic dermoscopic features.
