Abstract

Dermatosis papulosa nigra is a chronic skin condition that
affects dark skinned adults. Incidence in black population is
between 35-77% with women being more commonly affected than
men. About one –half of patients have positive family history.
Lesions appear as verrucous hyperpigmented papules from
1-5 mm on face, neck and upper trunk. Lesions affect more
photoexposed areas. DPN is considered to be a form of SK.
Pathology of DPN is similar to the acanthotic type of Seborrheic
keratosis but DPN is smaller showing parakeratosis, acanthosis,
hyperpigmentation, thick interwoven tracts of epithelial cells and
horn cysts.
DPN must be differentiated from SKs and its clinical
variants as stucco keratosis, inverted follicular keratosis, large cell
acanthoma, clear cell acanthoma and flat seborrheic keratosis.
DPN clinically resemble other pigmented skin lesions as
epidermal nevus, actinic keratosis, lichenoid keratosiss, solar
lentigens and melanocytic nevus. Pedunculated DPN resembles
acrochordon.
DPN is asymptomatic, not associated with any mortality or
morbidity. Patients often seek treatment for cosmetic reasons. A

variety of modalities can be used in treatment of DPN including
cryotherapy, electrodessication, curettage and scissoring. Different
types of lasers can be used as 532diode laser, KTP, long pulsed
Nd:YAG laser, Er:YAG laser, Pulsed dye laser and fractional
photothermolysis.
Er:YAG laser is a solid state laser. There are different types
of Er:YAG laser as long-pulsed Er:YAG laser, dual-mode
Er:YAG laser, short-pulsed Er:YAG laser and fractional Er:YAG
laser. Er:YAG laser is used in treatment of several skin conditions
as: compound nevi, verrucous epidermal nevus, milia,
xanthelasma, sebaceous hyperplasia, syringomas, trichoepitheliomas,
miliary osteomas, periocular milia en plaque,
cutaneous lesions of Hailey-Hailey disease and Darier’s disease.
Er-YAG laser is very effective in treatment of planter warts.
The Nd:YAG Laser is the most important solid-state laser.
It covers a wide range of medical applications. The Nd:YAG laser
is safe and effective laser for hair removal. Also it is useful in
treating vascular lesions such as blue spider veins, pyogenic
granuloma, erythematous striae, hidradenitis suppurativa, verruca
vulgaris, multiple cylindroma of the scalp, keratosis pilaris and
angiokeratoma of Millebi. The Nd:YAG laser has also been used
for basal cell carcinoma, squamous cell carcinoma, and malignant
melanoma.
